Thank you for choosing Rainier Physical Therapy for your rehabilitation needs. We’re committed to ensuring new physical therapy patients in Cranberry Township, PA, experience a first visit that is both comfortable and informative. We’re honored to be a part of your healing process.
Please arrive to your initial evaluation 15 minutes early to ensure that all of your paperwork is completed and processed and remember to wear loose, comfortable clothing. Additionally, please make sure to bring the following with you to the first appointment:
- Photo ID
- Insurance Card
- Provider Referral/Prescription
- Intake Form
We look forward to meeting you in person! Please contact us with any questions or concerns you have. You can also look over the Notice of Privacy Policy. In the event that you are unable to attend this or any future appointments, we kindly ask for a minimum of 24 hours’ notice.
For your convenience, our patient registration forms are available for printing. Please complete the first three forms and Physical Therapy Questionnaire above and bring all of them to your first office visit.
Insurance Information
We are contracted with most major insurance companies and will be glad to answer any questions you may have about your benefits. Don’t hesitate to contact our clinic at (724) 590-0494 for assistance if you don’t see your insurance below or if you need specific information about our billing process. Remember, while you are free to seek physical therapy in Pennsylvania for most insurances without a physician referral, you may still need one for certain plans.
